I know in the Antiochian church we are totaly familiar with the 40 days after death & many people hold memorial services for departed family members by this. I even knew of the 40 days well before becoming Orthodox within my family since my father was cradle Orthodox. The toll house concept seems alien to our tradition, I have spoken to cradle Orthodox American & old country Syrians who have no concept of it. It is not found in the formal theology of St. John of Damascus & the hymnography "attributed" to him with toll houses is suspect.

In its more developed concept, as propagated by Blessed Fr. Seraphim Rose among others, it's basically a late Russian theologoumenon which has spread in recent years, though often in a more moderate form.
