There are already two deacons in the icon or are those angels? They have the same clothes as you see in icons of St. Stephen the Protomartyr.

I believe a deacon (or even deaconess, at one time) could distribute Communion. However, even though the Theotokos gave birth to Our Lord, there's no evidence she was ordained a deacon. The person with the Communion spoon looks exactly like they make the Theotokos look in other icons, as far as the way she's dressed, so I assume it's her.
